High Holiday Guest and Request Form 2021

PLEASE USE THIS FORM TO ARRANGE FOR GUESTS TO ATTEND SERVICES ON THE HIGH HOLIDAYS AND FOR OTHER SPECIAL REQUESTS.
GUEST TICKETS
Guest Policy & Prices:
1. Local Guests - Living within 20 miles of B'nai Jacob - $150 per person per service
2. Non-local Guests - Living further than 20 miles of B'nai Jacob - $72 per person per service
3. Day two Rosh Hashanah is complimentary for guests, though registration is still required.
4. Teen and Adult Children (and their spouses) 29 years old or younger are FREE with your membership - no registration required.
5. Young Children - ages 3 - 12 years old are FREE.  Children's programming will be offered during services for members and non-members.  Email office@bnaijacob.org to sign up guest children (include name, age, and grade).  No sign up required for member children in Yahad or Ezra.
6. Courtesy Guest Passes are offered for guests upon receipt of written (emailed) confirmation of membership at another synagogue.
See below to request that CBJ send confirmation of your membership to another synagogue where you will be a guest.

- Name(s) of adults and children requesting tickets
- Service(s) attending (RH1, RH2, etc...)
- Name of you member hosts
